4. To avail of the promo, subscriber should submit:
   • Duly filled-up and signed application form
   • Photocopy of valid ID (original to be presented)
   • Most recent non-SKY Fiber internet provider bill or statement of account – wired/wireless and postpaid:
     • The name reflected on the non-SKY Fiber bill must be the same name to be used for the subscriber’s SKY Fiber account.
     • Must not be more than 60 days
     • Bill should show that last bill has been paid
5. Other existing policies and conditions for new subscriptions will apply.
6. Amount of free installation is non-transferable and non-convertible to cash.
7. This promo cannot be availed in conjunction with other SKY installation promos.
8. SKY employees, its agencies and affiliates including their relatives up to the second degree of consanguinity or affinity are disqualified from availing of this promo.
