5. To avail of the promo, subscriber should submit the following:
   1. Photocopy of valid ID (original to be presented)
   2. Cash-out equivalent to the offer being availed
   3. Filled-up and signed service application form
6. The free months’ subscription will be equivalent to the amount initially paid. If the subscriber changes his plan with a different monthly rate, the number of free months will adjust accordingly.
7. Amount of free months is non-transferable and non-convertible to cash.
8. This promo can be availed together with the free installation promo, if the subscriber meets its requirements.
9. Availers of this promo can only enroll under the monthly payment scheme. Should subscriber want to enroll to advance payment scheme, the free months should be fully consumed first.
10. The free months will be forfeited in case of permanent disconnection. Unused months cannot be refunded nor transferred to other account.
11. Pre-termination fee is ₱2,000 for standalone plans and ₱4,000 for bundled plans.
12. SKY employees, its agencies and affiliates including their relatives up to the second degree of consanguinity or affinity are disqualified from availing of this promo.
